UNDERWRITING - Disclosure of Financial Interest Form (“DS-1”), Illinois Department of Financial and Professional Regulation-Division of Financial Services
Dear Associates:
On March 3, 2022, an Illinois court issued a temporary restraining order in connection with the new DS-1 form that went into effect on March 1, 2022. Until further instruction is provided by the court, all Illinois issuing offices should continue to use and file the previous version of the DS-1 form for all applicable transactions. For reference, the correct form is included as an attachment to this bulletin.
