The Colorado Division of Insurance ("DOI") has directed that all title insurance policies issued shall include a written statement that lists any simultaneously issued endorsements. In order to comply with this directive, we have prepared and attached a form of written statement that is to be sent to the insured with their title policy. The form of the policy itself and any endorsements to the policy together with the respective charges should be set forth on this written statement. Effective immediately, this form should be attached in front of the policy at the time it is mailed to the insured. Additionally, you must retain a copy of this index in your title file.
